CD28分子存在于大多數T細胞表面，被認為是一種T細胞*的表面分子。T細胞對抗原的反應性主要通過CD3/TCR復合物介導，但也有賴于T細胞表面其他分子的協同。ＣＤ２８與其天然配基Ｂ７結合可發揮共刺激作用，作為第二信號介紹Ｔ細胞激活，促進Ｔ細胞增殖及分泌多種細胞因子并介紹導Ｔ，Ｂ細胞粘附。在體外T淋巴細胞激活擴增中抗人CD28單克隆抗體通過第二信號通路發揮共刺激作用。
